School Health
B’nai Shalom remains committed to keeping our school a safe and healthy environment for our students and staff. Our goal is to keep school operations and instruction calibrated to public health conditions in NC, specifically in Guilford County. We will continue to rely on the guidance, consideration, and mandates established by national, local, and state health experts and leaders.
One of the most important pieces to keeping our school community safe and healthy is to keep students at home when they are ill. Please notify your students classroom teacher when your child is home due to illness. Clear communication will ensure that your child returns to school as quickly as possible. If a student becomes sick while at school, they should be picked up within 30 minutes of notification. Below you will find quick reference sick policies, but please refer to your Parent Handbook for more details.
-

K-8 Health Forms
A medication form is needed for ALL medications (prescription, emergent, and non-prescription). We cannot administer any medication at school until we have the form completed in its entirety. The form MUST be signed by both a parent/guardian and a physician/healthcare provider in order for students to receive medications at school. All medications must be supplied by the family and all prescription medications must be in the original pharmacy container with the label.
Preschool Health Forms
Each Preschool student will need a completed Preschool Medical Form and an updated Immunization Record (an immunization printout from your pediatrician is preferable).
In addition, any Preschool student that will need to take a medication at school needs a completed Medical Action Plan and Medication Administration Form as mandated by the State. B’nai Shalom will need a separate Medication Administration Form for each medication.
